📖 Contenu du Protocole
Critères d’inclusion :
Douleur ou malaise d’origine non traumatique persistant ou disparu dans la région entre l’ombilic et la mâchoire
incluant le dos et les bras ou toute douleur angineuse habituelle ayant été à l’origine de l’appel ou suivant l’appel
des SPU.
1. Se référer au protocole 1 RÉP/ACP (Approche clinique préhospitalière).
2. Si fréquence respiratoire < 8/min, assister la ventilation avec de l’oxygène à 10 L/min ou plus.
3. S’assurer que le patient cesse toute activité.
4. Administrer de l’oxygène avec un masque de haute concentration à 10 L/min ou plus.
5. En présence d’une douleur thoracique, si le patient désire prendre sa nitroglycérine, valider les
critères d’exclusion pour la nitroglycérine :
a) En l’absence de critères d’exclusion;
• Installer le patient en position de confort;
• Évaluer la sévérité de la douleur avec l’échelle;
• Indiquer au patient que la prise de nitroglycérine doit se limiter à 1 jet de 0,4 mg sublingual (SL) ou
lingual (L) chaque 5 minutes;
• Surveiller l’intervalle entre les doses. Si la douleur est toujours présente, vérifier que le pouls radial est
présent et > 50/min et < 100/min
b) En présence de critères d’exclusion.
• Déconseiller le patient à prendre sa nitroglycérine;
• Installer le patient en position de confort.
6. Poursuivre le protocole 1RÉP/ACP (Approche clinique préhospitalière).
7. À l’arrivée des techniciens ambulanciers paramédics, leur transmettre l’information pertinente et
effectuer le transfert de responsabilités.

Critères d’exclusion pour la nitroglycérine :
État de conscience à « V », « P » ou « U »;
Absence de pouls radial;
Pouls < 50/min ou > 100/min;
Prise de médication pour la dysfonction érectile de classe PDE5;
• sildénafil (Viagra) < 24 heures;
• vardenafil (Levitra ou Staxyn) < 24 heures;
• tadalafil (Cialis) < 48 heures.
Prise de médication pour le traitement de l’hypertension pulmonaire :
• sildenafil (Revatio) – en tout temps;
• ratio-sildenafil – en tout temps;
• tadalafin (Adcirca) – en tout temps.
Autres médicaments de la même classe;
Tout supplément visant à améliorer la fonction sexuelle;
Douleur d’origine traumatique;
Grossesse ou allaitement;
Syncope associée à la douleur thoracique;
Présence de DAVG.
Remarques :
Aux fins de ce protocole :
• En présence de critères d’exclusion pour la nitroglycérine, transmettre au meilleur de ses connaissances
les conséquences possibles d’utiliser de la nitroglycérine avant l’arrivée des techniciens ambulanciers
paramédics. Si le patient souhaite ignorer les recommandations qui lui ont été adressées, le premier
répondant doit respecter la décision du patient et transmettre l’information aux techniciens ambulanciers
paramédics et bien documenter la situation.
• En l’absence de pouls radial, l’administration de la nitroglycérine doit être déconseillée pour la durée
complète de l’intervention. Si le pouls < 50/min ou > 100/min, l’administration de la nitroglycérine doit être
déconseillée pour la durée complète de l’intervention.

including the back and arms or any usual anginal pain that was the cause of the call or following the call
SPUs.

3. Ensure that the patient stops all activity.
4. Administer oxygen with a high concentration mask at 10 L/min or more.
5. In the presence of chest pain, if the patient wishes to take his nitroglycerin, validate the
exclusion criteria for nitroglycerin:
a) In the absence of exclusion criteria;
• Place the patient in a comfortable position;
• Evaluate the severity of pain using the scale;
• Inform the patient that taking nitroglycerin should be limited to 1 jet of 0.4 mg sublingual (SL) or
lingual (L) every 5 minutes;
• Monitor the interval between doses. If pain is still present, check that the radial pulse is
present and > 50/min and < 100/min
b) In the presence of exclusion criteria.

Taking medication for the treatment of pulmonary hypertension:
• sildenafil (Revatio) – at all times;
• ratio-sildenafil – at all times;
• tadalafin (Adcirca) – at all times.
Other drugs of the same class;
Any supplement aimed at improving sexual function;
Pain of traumatic origin;
Pregnancy or breastfeeding;
Syncope associated with chest pain;
Presence of LVAD.
Notes:
For the purposes of this protocol:
• In the presence of exclusion criteria for nitroglycerin, transmit to the best of your knowledge
the possible consequences of using nitroglycerin before the arrival of ambulance technicians
paramedics. If the patient wishes to ignore the recommendations addressed to him, the first
responder must respect the patient's decision and transmit the information to the ambulance technicians
paramedics and carefully document the situation.
• In the absence of a radial pulse, the administration of nitroglycerin should be discouraged for the duration
complete intervention. If the pulse < 50/min or > 100/min, the administration of nitroglycerin must be
not recommended for the entire duration of the procedure.
